Founded in 2007, ProHealth Home Care is a locally owned, Medicare-certified provider of care at home. From five locations, its clinical teams serve more than 2,500 patients across 13 counties in Northern California and Nevada with home health, hospice, rehabilitation and palliative care.
Preventive care is delivered daily by interdisciplinary teams — physicians, registered nurses, physical, occupational and speech therapists, medical social workers, dietitians and home health aides. AI-assisted workflows keep referrals, scheduling, documentation and compliance on track, so clinicians can spend each visit focused on the patient.
As a CMS-contracted provider, ProHealth employs more than 500 clinical and operational staff and collaborates with some of the most renowned hospitals and healthcare groups in the country.
